His work has been key in facilitating access to essential medicines, such as antiretrovirals for HIV, for underprivileged individuals. His commitment to public health led him to participate in the second edition of the Revolution conference, a space dedicated to analyzing current advances and challenges in HIV treatment and prevention.<\/strong><\/p>\n<p>The Revolution conference is organized by<a href=\"https:\/\/scienhub.org\/en\/activity\/revolution2025-en\/\"> <strong data-start=\"826\" data-end=\"848\">ScienHub Education<\/strong><\/a>, the training services platform of the <a href=\"https:\/\/lluita.org\/en\/?swcfpc=1\"><strong data-start=\"888\" data-end=\"929\">Fight Infections Foundation<\/strong><\/a>, with the aim of bringing together experts from various centers and hospitals to discuss key issues related to HIV. On this occasion, one of the invited speakers was <strong data-start=\"1096\" data-end=\"1121\">Mr. Alejandro Gaviria<\/strong>, a Colombian economist, engineer, and writer, with whom we had the pleasure of speaking to gain insight into how he tackled the challenge of leading the Ministry of Health and Social Protection in his country.<\/p>\n<h4><strong>As a former Minister of Health of Colombia, what do you consider to be the main current challenges in the fight against HIV in Latin America, and what strategies do you believe would be most effective in addressing them?<\/strong><\/h4>\n<p>During the six years I served as Colombia\u2019s Minister of Health (2012-2018)\u2014making me the longest-serving minister in that position\u2014I prioritized tackling two major challenges: <strong data-start=\"1748\" data-end=\"1772\">access to medication<\/strong> and <strong data-start=\"1777\" data-end=\"1834\">the financial sustainability of the healthcare system<\/strong>.<\/p>\n<p data-start=\"1839\" data-end=\"2208\">On one hand, ensuring that the system had sufficient resources to meet the population\u2019s needs was a crucial task. On the other, securing equitable access to medicines for all citizens, regardless of their socioeconomic status or place of residence, was a tremendous challenge in a country with deep territorial inequalities between urban centers and peripheral areas.<\/p>\n<p data-start=\"2210\" data-end=\"2354\">Undoubtedly, these were complex challenges, but they were also essential for advancing toward a fairer and more sustainable healthcare system.<\/p>\n<p><strong>Based on your experience, how has public policy influenced accessibility and equity in antiretroviral treatments in middle- and low-income countries?<\/strong><\/p>\n<p>In Latin America and low-income countries, <strong data-start=\"2565\" data-end=\"2630\">public health policies have been strengthened in recent years<\/strong>. I believe there have been undeniable advances in many health indicators, as well as in access to essential medicines, including antiretrovirals. However, the main challenge now lies in <strong data-start=\"2817\" data-end=\"2851\">incorporating new technologies<\/strong> and managing the growing financial pressure they impose on healthcare systems.<\/p>\n<h4>The big question is: <strong data-start=\"2955\" data-end=\"3086\">how can an innovative drug be integrated into the healthcare system in a way that makes it accessible to the entire population?<\/strong><\/h4>\n<p>Colombia\u2019s case is particularly complex. In my country, <strong data-start=\"3146\" data-end=\"3200\">about 50% of workers do not have formal employment<\/strong>, meaning their access to social security is not always guaranteed. One of our biggest challenges was ensuring that this large sector of \u201cinformal\u201d workers could be included in the public healthcare system, with the same coverage and rights as the rest of the population.<\/p>\n<h4><strong>Given your focus on democratizing access to treatment, what lessons learned in Colombia could be applied globally to improve care for people with HIV and other diseases?<\/strong><\/h4>\n<p data-start=\"3661\" data-end=\"3840\">I return to a theme I have already mentioned: <strong data-start=\"3707\" data-end=\"3755\">technological pressure on healthcare systems<\/strong>. A clear example of this challenge is the case of <strong data-start=\"3806\" data-end=\"3837\">medications for Hepatitis C<\/strong>.<\/p>\n<p data-start=\"3842\" data-end=\"4068\">When we analyzed the situation in Colombia, we quickly calculated how many people needed access to this treatment. We soon realized that if we had treated all patients at once, <strong data-start=\"4019\" data-end=\"4065\">the healthcare system would have collapsed<\/strong>.<\/p>\n<p data-start=\"4070\" data-end=\"4419\">So, how do we effectively manage these transitions? <strong data-start=\"4122\" data-end=\"4183\">Addressing technological pressure was a crucial challenge<\/strong>. Although we successfully implemented a strategy, it was a fragile victory: the introduction of new medications can create <strong data-start=\"4307\" data-end=\"4342\">financial sustainability issues<\/strong>, ultimately threatening the hard-earned progress in <strong data-start=\"4395\" data-end=\"4416\">healthcare equity<\/strong>.<\/p>\n<p data-start=\"4421\" data-end=\"4636\">In fact, the healthcare system has advanced significantly in terms of social equity, but the constant arrival of new drugs presents an ongoing challenge: <strong data-start=\"4575\" data-end=\"4633\">ensuring fair and equitable distribution to all people.<\/strong><\/p>\n<h3 data-start=\"4643\" data-end=\"4804\"><strong data-start=\"4647\" data-end=\"4802\">In your opinion, what are the most significant barriers to eradicating HIV, and how can they be overcome from both a scientific and social perspective?<\/strong><\/h3>\n<p data-start=\"4806\" data-end=\"4852\">In Colombia, we face <strong data-start=\"4827\" data-end=\"4849\">two major barriers<\/strong>.<\/p>\n<ol data-start=\"4854\" data-end=\"5083\">\n<li data-start=\"4854\" data-end=\"4995\"><strong data-start=\"4857\" data-end=\"4912\">Identifying individuals who do not have formal jobs<\/strong> and are therefore <strong data-start=\"4931\" data-end=\"4992\">not properly integrated into the public healthcare system<\/strong>.<\/li>\n<li data-start=\"4996\" data-end=\"5083\"><strong data-start=\"4999\" data-end=\"5080\">Ensuring that these people have effective access to antiretroviral medication<\/strong>.<\/li>\n<\/ol>\n<p data-start=\"5085\" data-end=\"5408\">Additionally, <strong data-start=\"5099\" data-end=\"5122\">treatment adherence<\/strong> remains a challenge. In some cases, <strong data-start=\"5159\" data-end=\"5219\">patients do not follow their medication regimen properly<\/strong>, and some even <strong data-start=\"5235\" data-end=\"5283\">resell their medicines on the illegal market<\/strong>, which further complicates the problem and undermines efforts to guarantee equitable access and effective disease control.<\/p>\n<h4><strong>Could you share a personal or professional experience that has shaped your perspective on the fight against HIV and other infections, as well as the importance of health equity?<\/strong><\/h4>\n<p data-start=\"5604\" data-end=\"5875\">Personally, I went through a <strong data-start=\"5633\" data-end=\"5668\">unique and difficult experience<\/strong>: <strong data-start=\"5670\" data-end=\"5738\">while serving as Minister of Health, I was diagnosed with cancer<\/strong>. This put me in an unusual dual position\u2014leading <strong data-start=\"5788\" data-end=\"5814\">public health policies<\/strong> while undergoing <strong data-start=\"5832\" data-end=\"5872\">intensive treatment for seven months<\/strong>.<\/p>\n<p data-start=\"5877\" data-end=\"6121\">It was an extremely complicated situation because, in a way, I became <strong data-start=\"5947\" data-end=\"5976\">&#8220;the patient&#8221; of Colombia<\/strong>. My illness was <strong data-start=\"5993\" data-end=\"6027\">followed by the entire country<\/strong>, almost like a soap opera, with media exposure that made the process even more challenging.<\/p>\n<p data-start=\"6123\" data-end=\"6506\">Regarding HIV, I have a <strong data-start=\"6147\" data-end=\"6172\">very close connection<\/strong> to this field. One of my <strong data-start=\"6198\" data-end=\"6214\">best friends<\/strong> is Colombia\u2019s leading HIV specialist, equivalent to <strong data-start=\"6267\" data-end=\"6293\">Dr. Bonaventura Clotet<\/strong> here. Ironically, when I was a cancer patient, <strong data-start=\"6341\" data-end=\"6376\">I nearly died from an infection<\/strong>\u2014I suffered <strong data-start=\"6388\" data-end=\"6404\">septic shock<\/strong>. And it was <strong data-start=\"6417\" data-end=\"6434\">precisely him<\/strong>, with his <strong data-start=\"6445\" data-end=\"6473\">experience and knowledge<\/strong>, who ultimately saved my life.<\/p>\n<h4>Finally, what message would you like to convey to healthcare professionals and researchers attending the &#8220;REVOLUTION \u2013 Advances and Challenges in HIV Treatment&#8221; conference?<\/h4>\n<p data-start=\"6697\" data-end=\"7167\">I would like to highlight some <strong data-start=\"6728\" data-end=\"6750\">bioethical debates<\/strong> I had to face during my time as Minister of Health in Colombia. These are fundamental questions that, despite their importance, are often <strong data-start=\"6889\" data-end=\"6925\">overlooked in public discussions<\/strong>. However, I firmly believe they should be at the <strong data-start=\"6975\" data-end=\"6997\">core of the debate<\/strong>, because many of the <strong data-start=\"7019\" data-end=\"7066\">sustainability issues in healthcare systems<\/strong> and the <strong data-start=\"7075\" data-end=\"7119\">challenges faced by low-income countries<\/strong> stem from unresolved <strong data-start=\"7141\" data-end=\"7164\">bioethical dilemmas<\/strong>.<\/p>\n<p data-start=\"7169\" data-end=\"7416\">For example, <strong data-start=\"7182\" data-end=\"7329\">how do we justify the inclusion of a new cancer drug that costs $1,000 per month and clinically only extends survival by an average of 25 days?<\/strong> How do we <strong data-start=\"7340\" data-end=\"7369\">ethically and responsibly<\/strong> make the decision to say \u201cno\u201d in such cases?<\/p>\n<p data-start=\"7418\" data-end=\"7641\">Another relevant dilemma is <strong data-start=\"7446\" data-end=\"7474\">healthcare service costs<\/strong>.
