This week, Dr Eug\u00e8nia Negredo<\/strong>, Head of the HIV Research Line<\/a> at the Fundaci\u00f3 Lluita contra les Infeccions, received the Prisma Grant<\/strong> from the biotechnology company Gilead<\/strong> <\/a>for Healthy Connections<\/em>, an innovative project aimed at improving the quality of life of people living with HIV.<\/p>\n

\n

Researcher Diana Hern\u00e1ndez<\/strong>, from the Foundation, will be responsible for the awarded project, Healthy Connections: Development of a territorial ecosystem for the comprehensive management of people with HIV<\/em>. The project proposes the creation of a territorial health and community ecosystem<\/strong> that enables a much more integrated, accessible and personalised approach to caring for people with HIV, especially those in situations of vulnerability such as women, people over the age of 55, and migrants.<\/p>\n

A pioneering and collaborative model of care<\/h3>\n

The initiative led by Dr Negredo represents a paradigm shift<\/strong> in HIV care: moving from a model centred exclusively on the hospital to a decentralised network<\/strong> that integrates:<\/p>\n

    \n
  • Primary care<\/li>\n
  • Community centres<\/li>\n
  • Social and support services<\/li>\n
  • Local organisations such as BCN Checkpoint, Panteres Grogues, among others<\/li>\n<\/ul>\n

    This strategic approach allows for the early identification of unmet needs<\/strong>\u2014medical, social, emotional or structural\u2014while providing responses through agreed-upon care pathways, interoperable tools, and the active participation of patients themselves as co-creators of the model.<\/p>\n

    Healthy Connections, a project with real impact on quality of life<\/h3>\n

    The programme is structured in several phases, including:<\/p>\n

      \n
    • Identifying unmet needs through focus groups, patient forums and validated screening tools<\/li>\n
    • Developing a dynamic map<\/strong> of healthcare and community resources<\/li>\n
    • Designing new multidisciplinary care pathways<\/li>\n
    • Evaluating the project through a pilot study to measure satisfaction, perceived experience and whether the identified needs have been addressed<\/li>\n
    • Developing a roadmap<\/strong> to replicate the model in other regions<\/li>\n<\/ul>\n

      The ultimate goal is to enhance satisfaction, empowerment and continuity of care<\/strong> for people living with HIV, strengthen treatment adherence, and reduce health inequalities.<\/p>\n

      A recognition of innovation and scientific commitment<\/h3>\n

      The Healthy Connections<\/em> project stands out for its community-based approach, its use of interoperable technology, and its preventive perspective, as well as for its potential to be scaled up in other autonomous communities.<\/p>\n
