The ”la Caixa” Foundation supports our MesAprop project

At the Fight Infections Foundation, we have been selected in the 2025 Catalonia Social Projects Call of the ”la Caixa” Foundation, presented on 4 June at an event attended by the beneficiary organisations.

The MesAprop project focuses on strengthening care for people aged over 50 living with HIV, a group that is particularly vulnerable to premature ageing, frailty and social isolation.

The initiative will roll out a comprehensive care model based on early detection and multidisciplinary psychological, physical and social interventions to improve participants’ quality of life, independence and overall wellbeing. The project has been designed to be scalable and replicable in other healthcare settings.

A person-centred care model

Through MesAprop, we aim to address a growing reality: we are witnessing the emergence of the first generation of older people living with HIV, whose needs are complex and extend far beyond medical treatment.

Our comprehensive model is based on a person-centred multidisciplinary care programme that combines a rapid and thorough assessment of each individual’s physical, emotional, psychological and social situation with a personalised evaluation of their needs.

Following this assessment, each participant is offered a tailored care plan, which may include physical exercise, psychological support, cognitive stimulation, health education and group activities. This approach allows us to provide continuous, coordinated support aimed at improving patients’ quality of life and independence.

Early detection and multidisciplinary intervention

One of the key elements of the project is the early identification of frailty and vulnerability, enabling us to intervene sooner and more effectively. To achieve this, we will implement new assessment tools that optimise clinical time and allow more people to benefit from the programme.

We will then deliver a range of coordinated interventions that address not only medical needs, but also issues such as mental health, social isolation and stigma, all of which are crucial factors in the wellbeing of people ageing with HIV.

With the support of the ”la Caixa” Foundation improving participants’ quality of life, strengthening their independence and advancing towards a more efficient, person-centred model of care.

MesAprop builds on our experience in HIV research and care and was created with the ambition of becoming a model that can be extended to other centres and regions, helping to transform care for people ageing with infectious diseases.
